Registered Nurse-Anticoagulation

The Anticoagulation Nurse provides competent care for all aspects of anticoagulation management for patients registered in the Holy Cross Anticoagulation Management Service. For daily operational supervision the position reports to the Anticoagulation Pharmacist, while working under the medical oversight of the clinic’s Medical Director.

Essential Job Functions
• Functions as the primary contact for all clinic patients, greeting all patients during their clinic visits
• Interacts with all patients with understanding, compassion and patient centered care
• Ensures patients are correctly registered in the HIS, collects and copies required documentation required for completing patient registration
• Performs limited physical assessments as required i.e. vitals including weights, BP
• Maintains an in-depth operational knowledge of coagulation point of care testing device including device maintenance and care, cleaning and running controls
• Performs patient point of care testing as required for treatment and monitoring of therapy
• Maintains required records for quality control testing, sample logs and temperatures
• Professionally interacts with referring/consulting providers regarding patient treatment and progress via effective written and/or verbal communications
• Has a demonstrated knowledge of the anticoagulation management software, able to schedule patients, document lab results, write notes etc. Ability to troubleshoot software program issues and refer to the vendor for follow-up and resolution as needed
• Educates patients regarding their treatment including therapy goals, duration of therapy, medications, potential side effects of treatment, complications and when to seek immediate medical care
• Performs billing entry for all clinic services
• Compiles required visit documentation and forwards to HIS for scanning and inclusion in the patient EMR
• Administers subcutaneous or oral medications as directed
• Maintains the clinic office space in a neat an orderly fashion to promote a professional image and reflect the care provided
• Orders or requisitions routine office or medical supplies
• Collects, documents or researches data for special projects/quality improvement efforts as requested to advance the practice and care delivery the Anticoagulation Management Service

• Graduate of an accredited school of Nursing


• Minimum of 1 year of experience a related clinical environment or acute care facility required, previous anticoagulation management experience preferred
• Ability to work independently with minimal supervision required
• Working knowledge of word processing and spreadsheet functionality (Word®/Excel®) required
• Effective written and verbal communication skills required
• Excellent customer relation skills in dealing with patients and coworkers required
• Professional collaboration skills with all members of the healthcare team required
• Bilingual in Spanish and English preferred

• Current nurse registration in New Mexico required
• Current BLS Certification or can be obtained from Holy Cross offered program within one year of employment is required
